Please instruct your child to notify his/her teacher any time he/she sustains a major or minor injury. An accident report form is required and will be filled out by the supervising teacher or educational assistant after an accident. Please be sure your child has a complete and accurate EMERGENCY CARD on file in the School Office. If during the school year, the phone numbers and/or information change, call us to update your child’s emergency card.  In case of emergency, we must have current information to be able to contact you during the school day if your child has incurred an injury. Parents will be notified of accidents as soon as possible.
